Train one on one with Carly
She introduces the apparatus, watches how you move with a few simple exercises, and guides your 55-minute workout from there.
Afterward, Carly tells you the strengths she noticed and what future sessions would focus on, and talks through a realistic weekly or monthly rhythm if you want to continue. You can also bring a partner later for a duet or trio session.
Who you’ll train with
Carly Jakubowski owns True Form. In a private session she watches how you move, cues you one on one, and adjusts the work as you go, so the session fits your body rather than a class plan.

What to wear and bring
- Bare feet or clean grip socks. Ordinary socks aren’t allowed.
- Comfortable clothing without zippers, buttons or snaps.
- Water, and arrive on time.
- Ages 15 and up. Ages 15 to 17 need a parent or guardian for the forms and first visit.

- All purchases are nonrefundable.
- Cancel or reschedule at least 24 hours ahead. Later changes and no-shows forfeit the session.
- Recent surgery or recent postpartum needs medical clearance. Questions? Contact Carly first.

Can I buy both intro options?
Yes. First-time clients may buy the single session, the three-session option, or both.
Do intro sessions expire?
Yes. Each intro purchase expires one month after purchase, and it can't be shared or transferred.
Is this a limited-time offer?
No. Both intro options are standing offers for first-time clients.
What if I have an injury or physical limitation?
Complete the health questionnaire and contact Carly first if you have questions. Recent surgery or recent postpartum status needs medical clearance.
What's the cancellation policy?
Cancel at least 24 hours ahead to keep your session. A later cancellation or a no-show forfeits it.
Which option do I pick at checkout?
Choose a time first. At checkout, select Intro Private Session ($75) or the 3-session intro ($195). You'll create or sign in to an Arketa account to finish booking.
What should I wear and bring?

What happens after my intro sessions?
If you want to continue, private sessions are $95, or $450 for five and $850 for ten. Five- and ten-session packs expire six months after your first session.
